The Hypermobility Hub specializes in comprehensive evaluation and care planning for individuals with hypermobility spectrum disorders, Ehlers-Danlos syndromes (EDS), and complex connective tissue conditions. Services include in-depth diagnostic assessments, telehealth consultations, second opinions, and ongoing care navigation designed to support patients with multisystem symptoms, chronic pain, joint instability, and autonomic dysfunction.

A Care Navigation retainer provides structured, ongoing support between scheduled visits for patients with hypermobility and EDS who need more than occasional appointments.
Many patients with complex connective tissue disorders require:

Rather than handling these needs informally or reactively, Care Navigation creates a clear, ethical, and sustainable framework for ongoing support.
What the Retainer Includes
Secure messaging access during business hours
Scheduled monthly check-ins
Review of labs or imaging obtained between visits
Coordination with physical therapy and specialists
Guidance during symptom flares and changes
What the Retainer Does Not Include
Emergency or urgent care
Unlimited visits
New diagnostic evaluations without reassessment
Prescriptions, procedures, or services outside scope
Care Navigation is optional, month-to-month, and available only after an initial evaluation.

Comprehensive Care Navigation – $500/month

Includes:
Priority messaging response times
More frequent asynchronous communication

The Hypermobility Hub
Appointments at The Hypermobility Hub are reserved specifically for you and include extended time for preparation, medical record review, and care planning. Because of this, the following policy applies to all appointments.
Cancellations & Rescheduling
Appointments must be cancelled or rescheduled at least 48 hours prior to the scheduled start time.
Cancellations made less than 48 hours before the appointment will incur a 50% charge of the scheduled service fee.
Same-day cancellations (including those made on the day of the appointment) will incur a 100% charge of the scheduled service fee.
No-Shows
A no-show is defined as:
Failure to attend a scheduled appointment, or
Failure to log into a telehealth visit within 10 minutes of the scheduled start time without prior notice.
No-shows will be charged 100% of the scheduled service fee.
Late Arrivals
Patients arriving more than 10 minutes late may have their appointment shortened or rescheduled at the provider’s discretion.
Late arrivals may still be subject to the full appointment fee.
Care Navigation Appointments
Missed or late-cancelled Care Navigation check-ins are considered completed services and are not refunded.
Monthly Care Navigation retainers are non-refundable once the billing cycle begins.
Repeated Cancellations
Repeated late cancellations or no-shows may result in:
Limited scheduling availability, or
Dismissal from the practice at the provider’s discretion.
Clinical Evaluation Services

Includes:
Focused medical history and symptom review
Guided hypermobility screening (including Beighton assessment when appropriate)
Review of prior diagnoses, labs, imaging, or genetic results (if available)
Education on hypermobility and self-management strategies
Written care recommendations and resource packet
15-minute follow-up call approximately 2 weeks later
Best suited for:
Recently diagnosed patients seeking management guidance
Patients with suspected hypermobility wanting expert input before pursuing further testing
Out-of-state patients seeking telehealth-only evaluation
Individuals with less complex presentations or limited budgets

Recommendations for appropriate laboratory testing or imaging (when applicable)
Comprehensive written treatment and care coordination plan
Referral guidance for physical therapy, specialists, or additional services
15-minute follow-up telehealth visit (~2 weeks)
60-minute follow-up visit (~12 weeks) to review results and progress
Best suited for:
Patients with suspected or confirmed EDS
Individuals with chronic pain, joint instability, autonomic symptoms, or multisystem involvement
Patients seeking diagnostic clarity and a long-term care strategy

Hypermobility Hub offers telehealth services in accordance with state licensure laws.
Clinical telehealth services are provided only to patients who are physically located in states where Crystal Wade, AGACNP-BC, holds an active license at the time of the visit. Telehealth visits may include follow-up care, care coordination, and ongoing management when permitted by state regulations.
For individuals who reside outside of licensed states, educational consultations may be offered. Educationalconsultations are informational in nature and do not include diagnosis, treatment, prescribing, or ordering of tests.
The Hypermobility Hub is designed for patients seeking thoughtful, comprehensive, and collaborative care. While we strive to be supportive and responsive, clear boundaries allow us to provide high-quality care sustainably and ethically.
The Hypermobility Hub provides services in accordance with Florida APRN practice regulations. Clinical services are available to Florida residents via in-person or telehealth visits, subject to licensure requirements.
Out-of-state services are limited to educational and consultative case reviews and do not establish an ongoing treating relationship.
The Hypermobility Hub does not provide emergency care. Patients experiencing urgent or life-threatening symptoms should contact local emergency services.
The Hypermobility Hub is a self-pay practice and does not bill insurance. This model allows for extended appointment times, comprehensive care planning, and individualized coordination without insurance restrictions.
Payment is due at the time of booking. Retainer services are month-to-month with no long-term obligation.
