The Hypermobility Hub specializes in comprehensive evaluation and care planning for individuals with hypermobility spectrum disorders, Ehlers-Danlos syndromes (EDS), and complex connective tissue conditions. Services include in-depth diagnostic assessments, telehealth consultations, second opinions, and ongoing care navigation designed to support patients with multisystem symptoms, chronic pain, joint instability, and autonomic dysfunction.

    All patients begin with a structured clinical evaluation (Tier 1 or Tier 2), ensuring appropriate assessment and care planning

    Step 2: Receive a clear, written plan

    Every evaluation includes education, recommendations, and next steps tailored to your presentation and goals.

    Step 3: Add ongoing support (Optional)

    Patients who need continued guidance between visits may enroll in Care Navigation for structured, ongoing support.

    Choose Tier 2: Comprehensive Hypermobility & EDS Evaluation ($600) if you:

    • Experience chronic pain, instability, autonomic symptoms, or multisystem involvement

    • Have seen multiple providers without clear answers

    • Need referrals, care coordination, or formal documentation

    Consider Care Navigation (Monthly Retainer) if you:

    Choose a Medical Case Review / Second Opinion if you:

  • A Care Navigation retainer provides structured, ongoing support between scheduled visits for patients with hypermobility and EDS who need more than occasional appointments.

    Many patients with complex connective tissue disorders require:

    Rather than handling these needs informally or reactively, Care Navigation creates a clear, ethical, and sustainable framework for ongoing support.

    What the Retainer Includes

    • Secure messaging access during business hours

    • Scheduled monthly check-ins

    • Review of labs or imaging obtained between visits

    • Coordination with physical therapy and specialists

    • Guidance during symptom flares and changes

    What the Retainer Does Not Include

    • Emergency or urgent care

    • Unlimited visits

    • New diagnostic evaluations without reassessment

    • Prescriptions, procedures, or services outside scope

    Care Navigation is optional, month-to-month, and available only after an initial evaluation.

  • Standard Care Navigation – $350/month

    Best for patients who:

    • Have moderate symptom complexity

    • Are relatively stable but benefit from regular guidance

    • Need periodic review and coordination

    Includes:

    • One scheduled monthly check-in (20–30 minutes)

    • Messaging support with standard response times

    • Limited documentation support

    Comprehensive Care Navigation – $500/month

    Best for patients who:

    • Have complex, multisystem involvement

    • Experience frequent or unpredictable flares

    • Require higher-touch coordination and advocacy

    Includes:

    • Priority messaging response times

    • More frequent asynchronous communication

    • Enhanced specialist coordination

    • Support with disability, accommodation, or medical necessity documentation

  • The Hypermobility Hub

    Appointments at The Hypermobility Hub are reserved specifically for you and include extended time for preparation, medical record review, and care planning. Because of this, the following policy applies to all appointments.

    Cancellations & Rescheduling

    • Appointments must be cancelled or rescheduled at least 48 hours prior to the scheduled start time.

    • Cancellations made less than 48 hours before the appointment will incur a 50% charge of the scheduled service fee.

    • Same-day cancellations (including those made on the day of the appointment) will incur a 100% charge of the scheduled service fee.

    No-Shows

    A no-show is defined as:

    • Failure to attend a scheduled appointment, or

    • Failure to log into a telehealth visit within 10 minutes of the scheduled start time without prior notice.

    No-shows will be charged 100% of the scheduled service fee.

    Late Arrivals

    • Patients arriving more than 10 minutes late may have their appointment shortened or rescheduled at the provider’s discretion.

    • Late arrivals may still be subject to the full appointment fee.

    Care Navigation Appointments

    • Missed or late-cancelled Care Navigation check-ins are considered completed services and are not refunded.

    • Monthly Care Navigation retainers are non-refundable once the billing cycle begins.

    Repeated Cancellations

    Repeated late cancellations or no-shows may result in:

    • Limited scheduling availability, or

    • Dismissal from the practice at the provider’s discretion.

Clinical Evaluation Services

  • This option is ideal for patients seeking expert guidance, validation, or a second opinion without the need for a full in-person evaluation.

    Includes:

    • Focused medical history and symptom review

    • Guided hypermobility screening (including Beighton assessment when appropriate)

    • Review of prior diagnoses, labs, imaging, or genetic results (if available)

    • Education on hypermobility and self-management strategies

    • Written care recommendations and resource packet

    • 15-minute follow-up call approximately 2 weeks later

    Best suited for:

    • Recently diagnosed patients seeking management guidance

    • Patients with suspected hypermobility wanting expert input before pursuing further testing

    • Out-of-state patients seeking telehealth-only evaluation

    • Individuals with less complex presentations or limited budgets

  • This is the flagship service of The Hypermobility Hub and is designed for patients with moderate to complex presentations requiring in-depth assessment and coordinated planning.

    Includes:

    • Comprehensive medical history and symptom analysis

    • Detailed joint and functional assessment

    • Review of prior labs, imaging, and specialist records

    • Recommendations for appropriate laboratory testing or imaging (when applicable)

    • Comprehensive written treatment and care coordination plan

    • Referral guidance for physical therapy, specialists, or additional services

    • 15-minute follow-up telehealth visit (~2 weeks)

    • 60-minute follow-up visit (~12 weeks) to review results and progress

    Best suited for:

    • Patients with suspected or confirmed EDS

    • Individuals with chronic pain, joint instability, autonomic symptoms, or multisystem involvement

    • Patients seeking diagnostic clarity and a long-term care strategy

  • This service is designed for patients who require ongoing guidance, coordination, and support between scheduled visits. It is available after completion of a Tier 1 or Tier 2 evaluation.

    Care Navigation provides structured access and professional oversight—it does not replace scheduled clinical visits or emergency care.

    Includes:

    • Secure messaging access during business hours

    • One scheduled monthly check-in (20–30 minutes, telehealth)

    • Review of outside lab results or imaging obtained between visits

    • Coordination with physical therapy and other treating providers

    • Assistance with care plans, flare management guidance, and next-step planning

    Includes everything in the Standard Retainer, plus:

    • Priority messaging response times

    • More frequent asynchronous communication as needed

    • Enhanced coordination with multiple specialists

    • Review of complex or evolving medical records

    • Assistance with disability, accommodation, or medical necessity documentation

    • Proactive flare-planning and care escalation guidance

    Best suited for:

    • Patients with complex EDS presentations

    • Multisystem involvement (autonomic, neurological, GI, autoimmune overlap)

    • Frequent flares or rapidly changing symptoms

    • Individuals requiring higher-touch coordination and advocacy

    Retainer services do not include emergency care, unlimited visits, new diagnostic evaluations without reassessment, or procedural services.

  • This service is available to patients outside of Florida or individuals seeking an expert second opinion.

    Includes:

    • Comprehensive review of medical records provided by the patient

    • Diagnostic insight and clinical interpretation

    • Education regarding condition-specific considerations

    • Discussion of treatment options and care pathways

    This service is consultative and educational in nature and does not establish an ongoing treating relationship. No prescriptions or orders are provided.

Insurance & Payment Policy

Hypermobility Hub offers telehealth services in accordance with state licensure laws.

Clinical telehealth services are provided only to patients who are physically located in states where Crystal Wade, AGACNP-BC, holds an active license at the time of the visit. Telehealth visits may include follow-up care, care coordination, and ongoing management when permitted by state regulations.

For individuals who reside outside of licensed states, educational consultations may be offered. Educationalconsultations are informational in nature and do not include diagnosis, treatment, prescribing, or ordering of tests.

The Hypermobility Hub is designed for patients seeking thoughtful, comprehensive, and collaborative care. While we strive to be supportive and responsive, clear boundaries allow us to provide high-quality care sustainably and ethically.

The Hypermobility Hub provides services in accordance with Florida APRN practice regulations. Clinical services are available to Florida residents via in-person or telehealth visits, subject to licensure requirements.

Out-of-state services are limited to educational and consultative case reviews and do not establish an ongoing treating relationship.

The Hypermobility Hub does not provide emergency care. Patients experiencing urgent or life-threatening symptoms should contact local emergency services.

The Hypermobility Hub is a self-pay practice and does not bill insurance. This model allows for extended appointment times, comprehensive care planning, and individualized coordination without insurance restrictions.

Payment is due at the time of booking. Retainer services are month-to-month with no long-term obligation.
