Our Process

    1. Before your visit.

    • Complete and submit the EDS Comprehensive Questionnaire at least 7 days before your visit.

    • Submit a Medical Records Release Form so relevant records may be requested in advance.

    • If you have previously been evaluated by Crystal in another practice or institution, please upload or bring your most recent clinic note.

    • Bring or upload relevant imaging reports and discs (MRI, CT, X-ray) when available.

    • Notify us in advance of any accommodations related to mobility, fatigue, pain, autonomic symptoms, or sensory sensitivities.

    2. Requirements to proceed with visit.

    EDS Comprehensive Questionnaire (Mandatory)

    To proceed with your scheduled consultation, the EDS Comprehensive Questionnaire must be fully completed and submitted at least seven (7) days prior to your appointment.

    This requirement allows adequate time to review your medical history by body system, identify patterns or red flags, and begin developing a preliminary care plan before we meet. If the questionnaire is not received at least one week in advance, your appointment may be postponed or rescheduled in order to maintain the quality and safety of care.

    3. What to wear to your visit.

    Please wear comfortable, loose-fitting clothing that allows for appropriate evaluation of the upper and lower extremities. In particular, we need to be able to observe and assess knee and shoulder movement during your visit.

    You are not required to remove any clothing. Your comfort, dignity, and sense of safety are always prioritized. Helpful examples include shorts or leggings, and tops that allow free shoulder movement.

    4. Preparing for a productive visit.

    EDS consultations are information-dense. Many patients experience brain fog, fatigue, anxiety, or symptom flares. Writing down your questions, concerns, and goals ahead of time can help ensure we focus on what matters most to you.

    • Up-to-date list of medications, supplements, and diagnoses

    • Written list of priority questions or concerns

    Important Reminders.

    Hypermobility Hub is a self-pay specialty practice and does not provide emergency or urgent care. Advance preparation is required for all EDS consultations. Incomplete pre-visit requirements may result in rescheduling in accordance with practice policy.
